Key Requests Policies

  1. All Key Request Forms must be signed by an authorized member of your department to be valid. Authorized members are individuals whose names appear on the signature authorization card as designated by your department. Invalid forms will be returned to the originating department. Please contact the Help Desk at 895-4357 should you need to fill out a card or amend your current card.

  2. Requests for lost or stolen keys/Marlok cards must be accompanied by a LOST/STOLEN KEY REPORT and an IDR. A $10.00 charge will be assessed per item unless the lost or stolen key was a master or sub-master. These prices can be found in the UNLV Campus Key Policy at facilities.unlv.edu. Please contact the help desk at (895-4357) immediately if lost or stolen keys are masters or sub-masters. Your department may be responsible for funding the re-key of the affected area or facility in these instances.

  3. Unwanted or obsolete keys, or keys from transferring or terminating employees should be returned to the Help Desk. This will end your department's responsibility for them.

  4. Duplicating or replacing keys through an outside agency, company, or private business other than UNLV is prohibited unless authorized in writing by the Lock Shop. Violations will be reported to the appropriate authorities. The duplication or possession of any unauthorized university key is considered a misdemeanor pursuant to NRS.205.080.

Locksmith Authorization Cards

In accordance with the published UNLV Key Policy, a Locksmith Authorization Card must be on-file for each department and section on campus.  These cards allow us to compare the signatures of authorized department representatives with those appearing on Key Requests as a means of security for our customers.  Keys will be made only for the authorized individuals as designated by the department head on the existing card.

Note: New cards must be submitted to Work Control as changes occur to those designated as authorized representatives in your department. Please contact the help desk at 895-HELP (4357).
